how to establish proof of india address with OCI ?

If I were to get an OCI and on visiting India, how do I get an proof of address document to get internet set up at my relative house, just not to bother using their ration card/voter card to get the internet connection ? any inputs ?

Affidavit.
Works for BSNL internet/mobile/PAN card etc, not for banks though, where you would need BSNL bill.

Can you please mention what is the format of this affidavit ? where is the template available? and who should sign it.
