I have applied for my wife and daughter on March 26th, 2012 and the approval received back to Hyd office on May 18th, 2012 (Roughly 7 weeks).
My application was sent on April 30th, 2012 and it is still under process (even after 10 weeks). I hope it gets better at Delhi office where they really get processed.

This might be a late reply but here you go:
[h=2]OCI Application Guidelines[/h]

FRRO provides a checklist but I have also included additional instructions.
Take originals of all documents whose copy you will be submitting.
You will be submitting 2 applications. So create 2 sets. Only set will include the demand draft.
2 copies of Part A and Part B for each applicant. Right thumb impression for female minors and left thumb impression for male minor who cannot sign. US passport size photo to be pasted on each of the 2 Part A forms.
Demand draft of 15000 INR for applicant with India visa. Demand draft of 8000 INR if the applicant is minor PIO card holder. Demand draft of 1400 INR if the applicant is adult PIO card holder. The demand draft should be from nationalised bank only. I used State Bank of India. The draft should be payable to 'Pay and Accounts Officer (Secretariat), Ministry of Home Affairs'. The draft is created by the bank in CAPITAL letter so the above does not fit so I had them create draft payable to 'PAY AND ACCOUNTS OFFICER (SECRETARIAT), M H A'. Also it is important to note that when you create the draft it should be payable at New Delhi branch
2 copies of each of the following (each set of the application will consist of one copy of each of the following):
Copy of foreign passport (photo page, visa page and last date of arrival stamp page). Write on the copy of the passport last date of arrival stamp page that this is the last date of arrival (DD/MM/YYYY).
Copy of certificate of mode of acquisition of present citizenship. Copy of birth certificate or naturalization certificate.
Copy of valid visa or PIO card with minimum validity of 31 days.
Copy of residential permit. Residential permit is not needed for minors. On the residential permit circle the date till which the permit is valid.
Evidence of Indian origin. Passport for self. For foreign born children attach the copy of photo page of foreign passport for father and mother. Mark on these copies that this is foreign passport copy of father and foreign passport copy of mother. Also include the copy of first and last page of most recent cancelled Indian passports of father and mother. Mark these copies accordingly.
Copy of first and last page of most recent Indian passport. Not applicable for foreign born children.
Proof of residence in India. Copy of rental/lease agreement or sale deed. Or Copy of utility bill. If the child's name is not on any of these documents, then in addition to these documents you will have to submit a notarized affidavit on a 10INR stamp paper that the child is staying with you. The format of this affidavit is specific. It is on another computer so I will post it later or email me for the affidavit format.
While not on the checklist I also included copy of renunciation certificate and the officer did not take it out. Renunciation certification is not applicable for minors who never had Indian nationality.

Hope this helps. Wife's application approved in 30 days. Mine and Child's application approved in 41 days. Wife's OCI received by HYD FRRO on 44th day.
Hi,
I am writing this appreciation for FRRO at HYderabad.
I did apply OCI for my whole family i.e me, my wife and two kids i.e handed over the application on Oct' 1st 2012. The OCI U visa was approved in Delhi on Oct 23rd and I got a SMS message on my mobile. On Nov 8th the OCI U visa has been arrived at Hyderabad FRRO office which is at Begumpet. Incredible! I can't believe it took less than 40 days!..
The people at OCI hyderabad are nice!. The only thing is we need to go there for the complete list as they don't have it on website and also they don't send it to your email when you request.. They will say come and collect it. Apart from this everything is good..

I applied for OCI on 5th november,2012 at FRRO, Hyderabad. But i live with my parents in Proddatur town of kadapa district, A.P. Even i am having the same problem address verification. I was told by the FRRO officer at Hyderabad that i don't require any kind of registration at FRRO/FRO(KADAPA DISTRICT) so i don't have any kind of registration certificate with me. Police called me two days back ago saying that i need to get certificate of residence from Mandal Revenue officer. Then the police will come from address verification at the place where i am staying now. It's after two weeks of applying did the person came to you for address verification or after 4 weeks. Did you recieve your OCI card with in the specified time or it took too long for you. If any one had gone through the same problem please help me.
