1. Birth Cert: You dont need to do anything for the Birth certificate. Just be ready with the name...I hope that you have done the search already for the name.
2. US Passport: Take the Birth certificate in original and photo of the baby (2-3) to nearest post office and apply for US Passport.... with a copy of birth cert as well. there will be a form in the USPS for passport application.
3. When you apply for passport, mention that you need SSN also... the SSN will be generated and sent home. along with the Passport. The original birth certificate will be retained by the post office.
All the 3 documents will be mailed to your home 1. Passport 2. SSN Card 3. Birth Cert....
Once you are done with the above 3, go to the indiam embassy and apply for the PIO card.
Dont forget to do the most important thing: That is to enjoy the moments with your new born. And take a lot of videos and snaps as well..

@geethbala.
Thanks for your input.
Actually we have made the SSN request in the hospital itself along with Birth Certificate. So do we have to wait for SSN before applying US Passport?.

We applied for Passport after getting the SSN. SSN from hospital may take around 6-8 weeks I guess.
Sears do a good job in getting the US passport specific photos for infants. Just check how many photos are needed for Passport and PIO. I don't remember if we took sheet of 4 or 8.
Apply for US passport with original birth certificate from hospital and from the county. Make sure you have copies of birth certificates before giving the original though they do send the originals.
Then apply for PIO.

For the thrifty
If you have a basic point&shoot digital camera, try using this website
http://www.epassportphoto.com/U can get pics of any passport/any purpose and print onyour printer/orders from walgreens/CVS. works out cheaper /free
but not sure what is needed specifically for a kids photo.

Just hold the baby with one hand at the bottom and one hand his head , point and click against a white background.
Passport pics for babies should be taken from 6-8 ft distance , as long as the face is visible along with some body you are good.
Print it at CVS for $0.6 ...you are done
US Passport office is flexible for baby pics
I did it and worked out just great
