What are the prospects for employment in the US someone with an MDS (mast. of dental science) degree from India. (without work experience)
I suppose some exams will have to be taken. what next after that?
Would this need an H1b visa? hence limited by the cap? Also whats the practicality of being hired by dental practices?

I met someone who's going through it. The person came to the U.S after BDS and went thru another 4 years of college here spending 150k. Currently "intern"ing with a desi dentist in Michigan.
To make it as a dentist in the U.S is tough.

What about visa status? If someone comes on a H-4 dependent visa? Are they affected by the h1b cap to when they want to work? do clinics who employ do H1b sponsorship?

India MDS cannot practice here. This will not buy you any concessions in gettting a BDS degree either.
MDS might help in clinical/lab work in Universities/Labs etc.

H4 to H1 conversion is still part of the H1 quota. It's tough to find someone willing to sponsor your H1.
The better option is for you to apply for GC and get to the I-485 stage. This will allow your spouse to get an EAD which makes it easier to work.
BDS or MDS ..It's a question of how many credits you get for your indian degree.. You need to pay for the remainder of the credits in the college. Either way you need to be ready to spend a nice chunk of money to get a US accredited Dentist degree.
