Form 8854 after abandoning green card

hello,

I am giving up my green card formally this week. In 2010, I was in the US for a period of 75 days or so. When filing form 8854, in Part 1 there are 2 questions - 'Expatriation Date' and 'Date of notification to homeland security'. Can the 'Expatriation Date' be the last date that I left the US and the 'Date of notification to homeland security' be the date when I formally give up the green card which is in 2days.
