I've got a few questions regarding the filing of schedule FA that I couldn't seem to find answers to. Any help would be greatly appreciated, thank you.
1. If I've got a 401(k) with dividend income in the previous year, do I fill that in under section B or section D of schedule FA?
2. How do I calculate investment at cost? All the money in the account is from employer contributions. The portfolio is held in a stable value fund (short term reserves) and no changes/contributions have been made by me. So is the investment at cost 0?

Gains is basically the difference between the sale amount and the amount that the employer pumped in. Then there is indexation.

I am not a CA, but I file returns on my own ITR2 as an NRI. The main section I fill is the short term capital gains that I make on sale of debt mutual funds in India. I have to file returns to get back the tds.

May be I am misunderstanding the whole question - but this seems to be a question related to "paper gain/loss" and not "realized gain/loss". Paper gains are not taxable - only realized gains are. Meaning, you took money out of 401k. Only if you take money out, will taxes be due.

#1. If dividends are shown in statement and then re-invested, those are taxable.
in Schedule - FA -> if you want to list each MF/holding, list in Part B otherwise, shown it as one line - 401-K retirement in Part D. Use cost basis in your statement and last purchase dates exg rate as purchase cost. Convert dividends to INR and show in appropriate columns and then also in OS-1b. I am not CA, but I have been doing this for last few years (and I use H&R block services for same).
And whatever you show as taxable amount(dividend), add that to cost basis when filing next year's return(if dividends are re-invested)
