Help with W8Ben form for 401k income

Hello all,

I have been asked to fill W8Ben form by Hewitt for my 401K income. In Part II, I have ticked Point 9a and 9b. Is that ok ?

What should I put in Point 10 ?

Help with this is *highly* appreciated.
