I do not know how the POA will be affected, or if it will be affected.

Personal suggestion - use the permanent Indian address. If you need to sign some papers, print them out (or have them sent over to you in US), sign them, either post them back or scan them for your father in India to print out. Your father can then post it to the PAN guys.

there are also a lot of agents that deal with PAN related issues, if that is the way you want to go, however, no personal experience with it.
