Just making sure about entering name correctly on OCI applicaiton

I am applying for my new OCI card...I already have renunciation certificate

On CKGS website it is asking first name(given name)(as on current US passport) and last name(given name) (as on current US passport)
I am confused about second part where it says first name (given name) .. on my passport it includes not only my first name but also include my fathers name as middle name. so what should I enter just first name or both first and middle name?

I just did mine. I included it identical to what's on my US passport. My given name (first and middle) on my US passport matched the first name on the OCI application as there is no middle name on the OCI application. It worked for me!

Thanks for quick relief.. I asked this question because when I printed 'additional particular form' it shows my first name as full first name+middle name...I am not sure if its their mistake where it should be given name instead first name...but

I have couple more questions...
1. on Govt. OCI application ..it is asking for applicant passport number..just making sure we are entering current US passport and not expired Indian passport correct?
2. On spouse details..my spouse is US citizen and not applying for OCI card so wondering if her passport details are necessary?
3. Also, it is asking for native address ...should I enter full address or just name of city?
and native means birth place or my last residence place in India ?
