I applied for OCI after acquiring my US Citizenship last year. My Indian passport at the front and back has a "cancelled", however does not say if "Cancelled due to foreign citizenship acquisition.
I sent email to the Indian consulate but there is no clear answer, they just forward a big FAQ, which does not have all the details and is confusing.
Do I need to apply for "Indian Citizenship Renunciation Certificate"? Has anybody done this yet?

On further thinking. While travel to India, we just need to carry the US passport and OCI (No Indian passport should be required, since it's cancelled), how should it matter, what's written on the canceled Indian passport?

IMHO, after they announced the Renunciation certificate rules, they started using this new Cancellation stamp. Our passports say "Cancelled due to foreign citizenship acquisition". If I had a OCI card, I would not worry about this cancellation stamp. The immagration folks at the airport do not know about this certificate. The certificate itself is a big joke. It is basically the form you send them with the same Cancellation stamp. Well worth the $175 :-)
If I am not mistaken, the US passport should have the OCI stamp/sticker. You will need to take your US passport and OCI card. There should not be any need to carry Indian passport.

I had the same concern.
I called both the FRRO in Chennai and the one in Bangalore. Both told me that if the passport was sent at some point in time to Indian Consulate for cancellation and if the passport has a stamp on it that says "Cancelled" there is no issue (no need to apply for a surrender/renunciation certificate).

Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20 only.
However US (foreign) citizens of Indian origin who naturalized on or after 1 June 2010, they have to follow the above-mentioned procedure with requisite fees of US$ 175/- for "Renunciation and Surrender Certificate"
