Repatriation and Tax

I read several threads regarding repatriating money from NRE into a US account, that it is possible but none of them discussed the actual procedure for doing it (what forms to fill, etc). If I missed any particular thread which has this information, I apologize.

Does anyone know of the procedure to repatriate money while in US from a NRE account in India? My NRE account is with SBI and I contacted the branch manager but waiting for his resposne. Their site is acting up lately as well (just when I need it.. :0))

Does anyone know how to do this ? Is there any tax we need to pay to IRS on the money we bring back the money to the US?
